5.3.16 
If there are any adverse changes to the conditions in the confined space or 
the time limit on the Permit to Work has expired, the work is to be stopped 
and the reasons reported to the Authorised Person. 
Cancellation of Permits to Work 
5.3.17 
On completion of the work, the Person in Charge is to: 
n 
withdraw all persons, equipment, tools and instruments from the 
point of work 
n 
advise all persons under his or her control that they are no longer 
permitted to enter the confined space 
n 
complete and sign Part 3 of the duplicate copy recording that the 
work has been completed  
n 
return the original Permit to Work to the Authorised Person. 
5.3.18 
The Authorised Person is to satisfy himself that the work has been 
completed satisfactorily. 
5.3.19 
The Authorised Person is to remove any keys, locks, signs or other safety 
equipment used for the work. Where associated Permits to Work, 
sanctions or other documentation have been issued, the Authorised Person 
is to liaise with the issuing officer for the re commissioning of any plant 
and equipment withdrawn from service. 
5.3.20 
The Authorised Person is to cancel the Permit to Work by destroying the 
original and completing Part 4 of the duplicate copy in the pad. 
5.3.21 
Completed pads of permits are to be placed in a folder marked  Permits to 
Work   Cancelled Duplicates  and retained in the Confined Spaces 
Document Cabinet for a minimum period of three years after the 
cancellation date of the last Permit to Work in the pad. 
5.3.22 
If the Person in Charge loses the original Permit to Work or his or her 
copy of the Safety Method Statement, work is to be stopped as soon as the 
loss is noticed. 
5.3.23 
When the work has been stopped due to loss of documentation, the loss is 
to be recorded by the Authorised Person on the duplicate copy. The Person 
in Charge is to countersign Part 4 of the duplicate copy to confirm and 
acknowledge the loss of the Permit to Work or Safety Method Statement. 
5.3.24 
The Authorised Person may stop the work if for any reason he considers it 
necessary. 
5.3.25 
Where the work is stopped by the Authorised Person or Person in Charge, 
the Permit to Work is to be withdrawn and cancelled.
