5.3.26
The Person in Charge is to :
n
withdraw all persons and, if safe to do so, all equipment, tools and
instruments from the place of work
n
advise all persons under his or her control that they are no longer
permitted to enter the confined space
n
take steps to prevent further access to the confined space and report
to the Authorised Person.
n
complete Part 3 of the duplicate copy of the Permit to Work
recording that the work has been stopped and the point of work has
been made safe
n
return the original Permit to Work to the Authorised Person.
5.3.27
Authorised Person is to:
n
destroy the original copy of the Permit to Work and complete Part 4 of
the duplicate copy recording that the work has been stopped and
follow up action will be taken
n
note the reasons for the withdrawal and any actions taken on the
original Safety Method Statement
n
issue a new Permit to Work before re starting work.
5.4
CONTROLLED ACCESS LOW RISK CONFINED SPACES
5.4.1
All persons entering a controlled access Low Risk confined space must be
supervised by a designated Person in Charge.
5.4.2
Before issuing the Safety Method Statement to the designated Person in
Charge the Authorised Person is to demonstrate:
n
the limits on the type of work that can be carried out without a Permit
to Work
n
the risk assessment and Safety Method Statement for the work to be
carried out
n
the logging in and out system, means of communication and
emergency rescue procedures.
5.4.3
Before accepting the Safety Method Statement the designated Person in
Charge is to:
n
read the Safety Method Statement
n
understand the limitation on work to be carried out
n
understand the safety precautions
n
countersign the original Safety Method Statement .
5.4.4
The acceptance of the Safety Method Statement makes the designated
Person in Charge personally responsible for monitoring work and ensuring
