that only work within the limits set by the Authorised Person is carried
out.
5.4.5
While the work is in progress, the Person in Charge is not permitted
to leave the point of work or to undertake any other work or tests. If
there is a need for the Person in Charge to carry out any other
unrelated duties or leave the point of work the task is to be
suspended. The Person in Charge is to ensure that suitable safety
precautions are taken before leaving the point of work.
5.4.6
If there are any adverse changes to the conditions in the confined space or
the work is to be stopped for any other reason the Person in Charge is to:
n
withdraw all persons from the place of work
n
advise all persons under his or her control that they are no longer
permitted to enter the Confined Space
n
take steps to prevent further access to the confined space and report
to the Authorised Person.
5.4.7
Where the work is stopped the Authorised Person is to:
n
note the reasons for the stoppage and any actions taken on the
original Safety Method Statement
n
re assess the levels or risk and issue a revised Safety Method
Statement and / or Permit to Work as appropriate.
5.5
CONFINED SPACES REGISTER
5.5.1
For each site, location or geographical area, a Confined Spaces Register is
to be kept by the Authorised Person. The Confined Spaces Register is to
comprise a series of folders and files containing information on confined
spaces and be kept in the Confined Spaces Document Cabinet.
5.5.2
The information must include:
n
an index of the information
n
a site plan / plans showing the reference number and location of the
confined spaces
n
a schedule of confined spaces with reference numbers, location,
general description, classification and known hazards
n
a general risk assessments for each confined space
n
log of entries effected.
