the detailed examination of a representative sample of the documents 
used with these Safety Rules and Procedures raised since the last 
major audit 
n 
a review of schematic drawings, training and refresher training, 
operational appointments and safety signs. 
9.2.2  
Authorising Engineers are to undertake an intermediate audit at each 
establishment on site nominally at six month intervals but not exceeding 
seven months from the last major or intermediate audit. 
9.2.3  
An intermediate audit is to consists of the random examination, review and 
inspection of documents raised since the last major audit and confined 
space classification.
9.2.4  
Authorising Engineers are to monitor the performance of Authorised 
Persons to ensure that these Safety Rules and Procedures are being 
correctly operated and administered. Monitoring is to be carried out at six 
monthly intervals and may be included in routine visits to establishments. 
The monitoring procedures are to include the following: 
n 
reviewing the operational experience and quantity of Authorised 
Person's duties 
n 
monitoring a random sample of the entries made in the Confined 
Spaces Register 
n 
monitoring the application of these Safety Rules and Procedures and 
their effectiveness 
n 
reviewing the Action Plan of the previous audit 
n 
bringing to the attention of the Co ordinating Authorised Person any 
uncompleted actions 
n 
discussing the reason for the delay and agreeing a revised programme 
for outstanding items 
n 
keeping a record of any routine visit or monitoring exercise. 
9.2.5  
At the discretion of the Authorising Engineer, a written report may be 
compiled and issued within 28 days of the completion of any site visit or 
monitoring exercise. A copy of the monitoring report is to be: 
n 
forwarded to the Co ordinating Authorised Person 
n 
placed in the Confined Spaces Document Register by the Co 
ordinating Authorised Person 
n 
retained by the Authorising Engineer. 
9.2.6  
It may be necessary for a revised Action Plan to be compiled by the Co 
ordinating Authorised Person. This revised Action Plan is to be agreed 
with the Authorising Engineer within 28 days of the issue of the 
monitoring report.
